How to Change the Password on the Saqsham Portal

What is SaQsham Portal? SaQsham is an online portal wherein Nationwide health centers and public health facilities can come up for national level certification. This portal has been developed for National Quality Assurance Programme under National Health Mission (NHM) with an objective of assuring Quality Services at Public Health Facilities, and also to improve it further for enhanced users’ experience at the facilities. This provides a step-by-step approach for conducting Quality assessment by providing a holistic view to QA assessors on methodology and scope of work.

Objectives

• To build an institutional framework mechanism for conducting Quality Assessments in health facilities.
• To maintain a repository of NQAS Certification.
• To guide assessors in understanding technical and managerial aspects of QA assessment.

How to Change the Password on the Saqsham Portal.

Step 1: First you have to go https://saqsham.nhsrcindia.org/IMCS/init then click on Forgot Password.

Step 2: Now you have to fill required data.

1. Select Type: Here you have to Choose Facility Incharge.
2. Enter User ID: Here you have to write your User ID. It looks like FA_MP_SC_UXXXXX, If you forgot your User ID, please check your registered Email.
3. User Mobile Number: Here you have to write your registered Mobile Number, which was submitted by you during Saqsham Registration Process.
4. User Email ID: Here you have to write your registered Email ID, which was submitted by you during Saqsham Registration Process.
5. Captcha: Write captcha, shown in Image.
6. Now click on Send OTP.
7. Check you registered Mobile and Email. Now verify both OTPs and click to Submit.

Step 3: Now open your registered Email, you’ll get new Email from NHSRC, Open it.

1. In this email you’ll find a PDF file, Open it.
2. Now enter Password on this PDF file (Starting 4 letters of your Email and Last 4 digit of you mobile number).
3. Now your PDF file is open and It shows your passwords.

Step 4: Now on the saqsham portal you’ll find a popup message, Click on OK.

1. Now write your User ID and Password and fill captcha and click to login.

Step 5: After Log In, You’ll get option to change Password.

1. Enter Old Password: Write a password, which one you read in PDF file.
2. New Password: Create New Password (Eg. Name@1234 )
3. Re-Enter Password: Repeat password for confirmation.
4. Now Click on Save
5. Your password Changed Successfully.

Step 6: Now you can Log In your SaQsham Portal and Check or Submit your NQAS Documents.
