This work instruction guide is designed to help Community Health Officers (CHOs), ANMs, MPWs, and ASHAs effectively manage Common Non-Communicable Diseases or NCD such as Hypertension, Diabetes, and Cancers (Oral, Breast, and Cervical) among individuals aged 30 years and above. It outlines each team memberโs role, where ASHAs are responsible for identifying eligible individuals, filling CBAC forms, raising awareness, and mobilizing the community for screening. ANMs and MPWs support ASHAs by supervising data collection, providing materials, and verifying the accuracy of records. CHOs conduct screenings, provide initial treatment, counsel patients on healthy lifestyles, and refer serious or complex cases to higher facilities. Screening is to be conducted every five years using specific methods: blood pressure checks for hypertension, blood sugar tests (RBS, FBS, PPBS, HbA1c) for diabetes, and OVE, VIA, and CBE for oral, cervical, and breast cancer respectively. Once diagnosed, patients are tracked for treatment adherence, counseled regularly, and referred to higher centers if needed. The overall objective is early detection, proper referral, and ongoing care to reduce disease burden and improve health outcomes in the community.

Who Should Use This:
ASHAs, MPWs/ANMs, and CHOs working at Sub Health Centres (SHC-HWCs). It explains your roles in identifying people aged 30+ years, filling risk assessment forms (CBAC), and ensuring proper referral and follow-up.
๐ฅ 2. How to Deliver NCD Services
At Community Level (by ASHA & ANM):
- Spread awareness during home visits or meetings.
- Help people adopt healthy lifestyles.
- List all people aged 30+ and assess their risk.
- Refer people with symptoms for further screening.
At SHC-HWC (by CHO, ANM, MPW):
- Screen referred patients.
- Dispense medicine as prescribed by the Medical Officer.
- Refer suspected cases to PHC/CHC.
- Keep track of follow-ups and treatment.

CHO:

๐ Hypertension (High Blood Pressure)
- Normal: Less than 120/80 mmHg โ suggest a healthy lifestyle.
- Pre-Hypertension: 120โ139/80โ89 โ advise on diet, exercise, and no smoking.
- Stage 1: 140โ159/90โ99 โ recheck in 1 month; refer to PHC if no improvement.
- Stage 2: 160โ179/100โ109 โ refer to PHC in 1โ2 weeks.
- Emergency: 180+/110+ โ check again in 15 mins and refer immediately.
๐ Diabetes
- Random Blood Sugar (RBS): If โฅ 200 mg/dl with symptoms โ Diabetes.
- Fasting Blood Sugar (FBS):
- 70โ110 mg/dl = Normal
- 110โ125 mg/dl = Pre-diabetes
- 126+ mg/dl = Diabetes
- Post Meal (PPBS): โฅ 200 mg/dl = Diabetes
- HbA1c: โฅ 6.5% = Diabetes
Types of Diabetes:
- Type 1: Usually in children; body makes no insulin.
- Type 2: Common in adults; insulin is low or not used properly.
- Gestational: Happens in pregnant women.

Signs of Possible Cancer:
- Long-lasting ulcers or swellings that donโt heal.
- Growths that bleed or change in shape.
- Any unusual lump that gets bigger or feels hard.
